Thank you for choosing our office for your healthcare needs. The following policies have been developed to help us serve you better:


PATIENT CONFIDENTIALITY

Ensuring patient confidentiality and the right to privacy is of the most importance to us. We are fully compliant regarding HIPAA. We ask that you designate a person to discuss your needs or concerns when you are not available.
We have designated a certain person for you to speak to with any concerns.


CONTACTING OUR OFFICE

Your call is important to us. You can reach us between the hours of 9am and 12noon and 1pm and 4pm on weekdays. For non-emergent calls, your call will be returned within 24 hours. Physicians may call you back later in the day or early evening after office hours. Urgent calls receive priority. Please go to the ER in case of an emergency.


APPOINTMENTS
Please arrive for all appointments on time. Patients will be seen in the order of their appointment. If you arrive more than 20 minutes late, you will be asked to reschedule. Please bring your insurance card and all your medications to each visit. We require 24 hours notice if you need to cancel your appointment. Late cancellations are considered a "no show", and there will be a fee of $25 charged to your account. We discharge patients with a poor attendance record or being noncompliant with the practice.


PRESCRIPTIONS

Request for refills require 24 hours notice. Please have the following information available:

  • Name of Drug
  • Dose
  • Name and phone number of the pharmacy
  • Name of person picking up the prescription
  • Phone number where we can reach you

Please check with your Pharmacy if your prescription is ready before calling us. By law, we are not able to phone in controlled drugs. All narcotic scripts much be picked up at our office. Routine refills are not available after hours. You must be an established patient to receive prescriptions. The doctor will not refill prescriptions without regular visits.


TEST RESULTS NOTIFICATION

We realize that you may be anxious to receive the results of your tests. We ask that you allow our office 4-5 days to process the reports. If you do not receive notification after this time period, please call the office. Test facilities may not report to us in a timely manner.


REFERRALS / AUTHORIZATIONS

Please allow us 2 business days to process your referral request. It is your responsibility to maintain an active referral. Prior authorizations may take longer. Please bring your referral to your first Pain Management appointment.


MEDICAL RECORDS RELEASE

Requests for medical records must be in writing, or signed by the patient or legal guardian. They will be ready in a week after receiving the request. New York State law allows us to charge 75 cents per page. Payment is required before releasing the records. Please note that the medical records are the Physician's property.


BILLING

Our practice participates in most major plans in the Capital Region. It is the patient's responsibility to verify that we participate in his/her plan. All copays are due at the time of the visit.


CELL PHONE USE

Please turn off your cell phone upon entering the office to avoid any interruptions in our service to you.
